How to start a career in coagulation - Trainees' Day

“How to start a career in coagulation” is a one-day workshop designed primarily for haematology trainees, but newly appointed haematology consultants would also be very welcome to attend. It is timed to take place just prior to the 2012 ISTH SSC in Liverpool and we hope attendees will also take advantage of the opportunity to register for the remainder of the SSC which looks set to be a stimulating meeting on a broad range of topics within haemostasis and thrombosis. “How to start a career in coagulation” is split into a morning session focussing on trainees in research, and an afternoon session focussing on common on-call clinical problems in the field, and delegates can register for either the morning or afternoon session or for the whole day. Registration for this day will be free based on generous educational grant funding from the British Society for Haematology and pharmaceutical support. Details of the registration process will appear at a later date.

World Haemophilia Day

Every year on April 17, World Hemophilia Day is celebrated around the world in order to increase awareness of hemophilia and other inherited bleeding disorders. World Hemophilia Day was started in 1989 and the World Federation of Hemophilia.

(WFH) chose April 17 in honor of WFH founder Frank Schnabel, who was born on that day.

National Children Education Workshop

Advances in medical procedures and therapeutics and the need to reduce patient length of stay are compelling the coagulation department to expand its routine test menu. Attend this session to gain a thorough update in this important area and discover how your laboratory can contribute to the best patient care.

You’ll begin with the basics: a review of hemostasis, including the synthesis and function of coagulation factors, followed by a discussion of current testing methodologies and instruments. Factor assays, von Willebrand testing, and platelet aggregation will be simplified to highlight the laboratory’s contribution to managing the bleeding patient. The focus will then shift to the thrombotic patient and a systematic approach to cost-effective testing for genetic and acquired disorders. You’ll participate in case studies that will reinforce the information presented and demonstrate how expanding the in-house coagulation menu can lower laboratory spending and reduce a patient’s length of stay. Additional case studies will illustrate the unexplained interferences that increasingly are affecting coagulation testing. Following this workshop, you will be able to:

• Describe the coagulation pathways and testing protocols for PT, PTT, Fibrinogen, D-Dimer and Anti-Xa.
• Implement simple algorithms to help the clinician better manage the bleeding patient.
• Discuss thrombophilia and identify cost-effective tests for your laboratory.
• Explain how bringing special coagulation testing in house can reduce laboratory costs and
patient length of stay.
• Identify recently occurring pre-analytic interferences.

The 9th INTERCEPT (INTERnational Course on Extracorporeal Perfusion Technologies) and Outcome Meeting will take place in Milan, April 12th -14th, 2012. Following the recently updated format, this Course maintains its identity of Perfusion-oriented meeting, but includes many other aspects related to the outcome of cardiac surgery patients. We have maintained the location downtown Milan, in the Congress Center of the Michelangelo Hotel, right in front of the Railway Station.

On day 1 we will start with the Opening Lecture “Towards new concepts in outcome measurement” by Sam Nashef. The father of the EuroSCORE and the New EuroSCORE, dr. Nashef is the perfect lecturer for opening an Outcome Meeting. Than the focus will continue on transfusions and coagulation. Last year we were focused on thrombin generation; as a natural consequence, this year we will go a step forward, concentrating on all the factors that contribute to clot strength and stability after thrombin generation. Session I will address Clot Firmess, and Session 2 the puzzle of Bleeding and Bleeding definition.

The focus on coagulation will continue on day 2 with the Coagulation Clinic, where the experts of the Faculty will present clinical cases to be debated with the audience.

Thromboprophylaxis in Obstetrics: A Workshop for Midwives and Obstetricians 5th October 2011

This meeting is part of a new UK Thromboprophylaxis Forum initiative for Midwives and Obstetricians.

Professor Beverley Hunt, Haematologist at Guy's and St Thomas's Hospital has taken the lead for the programme and along with Obstetrics colleagues and also the Royal College of Nursing and National Nursing and Midwifery Network, has come up with an afternoon to discuss issues within thromboprophylaxis.

The meeting promises to be very interactive and is aimed at consultants leading obstetric teams to midwives and anti-coagulation nurses from the unit.

The meeting will be held on the 5th October 2011 at 20 Cavendish Square, a really central venue in London with access from Oxford Circus or Bond Street stations. It will begin at mid-day with lunch and will finish at 5.30pm.

The goal of PRN is to address gaps in current training programs for HTC professionals. Building on the demonstrated success of other Partners in Bleeding Disorders Education programs, PRN utilizes web-based technology to offer online access to selected Partners in Bleeding Disorders Education educational activities. PRN will offer expanded educational content and resources that providers can access at their convenience. The content will include instruction on and support for projects and protocols such as the Universal Data Collection (UDC) project and other national priorities and initiatives.

WORLD FEDERATION OF HEMOPHILIA MUSCULOSKELETAL CONGRESS

The next meeting of the Musculoskeletal Committee of the World Federation of Hemophilia (WFH) will take place in Cairo, Egypt, from March 31-April 3, 2011.

The Musculoskeletal (MSK) Congress is a wonderful opportunity for those with an interest in the musculoskeletal aspects of hemophilia to meet and exchange experiences. Traditionally, attendees have included orthopedists, physiotherapists, physiatrists, hematologists, and scientists; however, we look forward to welcoming any and all who are interested in this area. There will be ample opportunity to share new ideas between peers, colleagues, and friends, and for establishing new contacts with people from all over the world.

Congress summary

The 12th International Musculoskeletal Congress is organized by the World Federation of Hemophilia (WFH) in collaboration with the Hemophilia Society of Egypt, and the Musculoskeletal Committee of the WFH.

This Congress brings together world-leading orthopedic surgeons, hematologists, and physiotherapists specialized in the treatment and care of patients with bleeding disorders. Participants will spend three days not only analyzing and discussing new medical developments but also looking at problems and issues in different parts of the world.

Sessions will focus on such topics as:

* repetitive psoas bleeds
* anesthesia in surgery for hemophilia
* peri-operative pain management
* post-operative regional nerve blocks
* hematology for the non-hematologist.

Course Overview

Scripps Cancer Center’s 31st Annual Conference: Clinical Hematology and Oncology 2011 is designed for hematologists, oncologists, surgeons, radiation oncologists, internists, and others desiring an update in these specialty areas. The primary objective of this course is to bring together clinicians and leading experts in diverse areas of hematology and oncology, affording them an intimate environment in which to discuss new clinical developments and significant advances.

The program focuses on selected hematologic subjects the first day, hematologic malignancies the second day, new developments in oncology with a focus on issues of current interest the third day and breast cancer the final day. Afternoon workshop sessions will be offered focusing on topics in hematology and coagulation. All presenters will discuss their approaches to challenging but common clinical situations and there will be ample time for participant interaction.

Advances in coagulation protein replacement therapy for people with congenital hemophilia A and B have led to progressive reductions in morbidity and increases in life expectancy. Recombinant and high purity plasma-derived factor products are currently safe and effective for the management of hemophilia. Modification of recombinant factors with enhanced biologic properties are being explored and show promise to improve hemophilia treatment. Gene therapy strategies to safely modify the hemophilia phenotype are in investigation and remain a long-term goal for clinical application. This session will focus on complications and comorbidities in the aging hemophilia patient, advances in coagulation protein therapies, and targeted gene therapy for hemophilia A and B. Dr. Claire Philipp will describe the changing demographics of hemophilia A and B and the impact of increased life expectancy on hemophilia-related complications and age-associated comorbidities. Challenges in hemophilia treatment and management of comorbidities in the older adult hemophilia patients will be discussed. Dr. Steven Pipe will provide an overview of new coagulation protein therapeutics bioengineered to increase circulating half-life, reduce immunogenicity, and improve the efficiency of biosynthesis, as well as other novel areas of investigation examining alternative therapeutic agents. Dr. Robert Montgomery will discuss recent data on targeted gene therapy of hemophilia A and B. Initial strategies focusing on the restoration of plasma factor VIII and factor IX and novel approaches targeting expression of factor VIII and factor IX in megakaryocytes and endothelial cells will be discussed. Studies in hemophilia mice demonstrating differences in clinical efficacy will be presented.

The approval of new oral anticoagulants that do not require laboratory monitoring will open a new era in the prevention and treatment of venous thromboembolism (VTE) and stroke prevention in atrial fibrillation. Challenges will occur when recurrences or major bleeds occur on the new agents. The post-thrombotic syndrome (PTS) is a common, chronic, burdensome, and costly complication of deep-vein thrombosis (DVT). There remains a continuing controversy regarding the need for long-term anticoagulation for patients with a first episode of unprovoked VTE. Dr. Jack Ansell will provide an update on the most recent results of phase III studies with new oral anticoagulants. Their significance and potential impact compared with standard warfarin therapy will be discussed along with how these new agents will affect the everyday life of physicians and patients. Dr. Susan Kahn will discuss the relationship between DVT and PTS, highlighting recent work on its epidemiology, classification, prevention, and management. Ongoing trials and areas for future research will also be discussed. Dr. Kenneth Bauer will present data regarding the recurrence risk in patients with a first episode of VTE and the benefits/risks of long-term warfarin therapy. Data regarding the use of the laboratory to risk-stratify patients with respect to recurrence will be discussed along with the impact of residual thrombus on imaging studies or having a non-removable inferior vena cava filter.

This session will focus on recent advances in the pathophysiology, assessment, and management of common disorders of platelet numbers and function, including immune thrombocytopenic purpura (ITP), thrombocytopenia during pregnancy, and qualitative platelet disorders. Dr. Adam Cuker will review recent developments in ITP, including changes in nomenclature and demographics, diversity in pathogenesis (including the role of infection, T-cells, and inhibition of platelet production), and recommendations from the International Consensus Report on diagnosis. He will review attempts to alter the natural history of ITP with new approaches to initial therapy, revisit splenectomy, and provide an update on the results of trials evaluating thrombopoietin receptor agonists. Dr. Keith McCrae will review the differential diagnosis of thrombocytopenia in pregnancy, including gestational thrombocytopenia, ITP, preeclampsia, HELLP syndrome, thrombotic microangiopathies, and less common disorders with a focus on recent developments in the pathogenesis of preeclampsia. Dr. Lawrence Brass will describe emerging ideas about platelet activation and thrombus formation at sites of vascular injury and disease. Focus will be on molecules on the platelet surface (some well known, others newly described) that regulate thrombus growth and stability. New ideas about the impact of antiplatelet agents will be discussed, as will congenital defects in platelet function and the evaluation of platelet function in the clinical setting.

The aim of this seminar is to offer general discussion on emerging clinical and scientific concepts in pulmonary fibrosis.

On the first day, we will evaluate advances in autoimmune and idiopathic fibrosis in parallel, with regard to both pathogenesis and treatment. The participants will be exposed to the latest thinking on diagnostic and prognostic evaluation, monitoring, and current and novel treatment approaches in idiopathic and autoimmune disease. Recent important clinical statements and studies will be covered in details.

On the second day, the emphasis will be on laboratory science with the central theme being research developments in the investigation of the pathogenesis of fibrosis. The aim is to make current thinking on key candidate pathways in fibrogenesis, including pathways of coagulation, epithelial mesenchymal transition and bone marrow involvement, accessible to participants.

Participants will gain knowledge from recent data to improve the routine investigation and management of their patients. Attendees will be able to develop their own research interests in clinical science and they will benefit from a working knowledge of the range of candidate pathogenetic pathways in pulmonary fibrosis.

This in-depth course will provide an overview and update of hemostasis, including coverage of coagulation and fibrinolysis testing, as well as a brief review of laboratory evaluation of the most common thrombophilic risk factors. Case studies will be used to demonstrate many of the assays and concepts. Discussions will include frequently ordered hemostasis assays, laboratory evaluation for lupus anticoagulants, and much more. You will leave with the ability to:

Identify hemostasis assays that should be performed in patients with a bleeding diathesis but normal APTT and PT assays and interpret the results.
Implement evaluation of an abnormal APTT and/or PT.
Identify the assays commonly performed to evaluate for thrombophilia.
Recognize the clinical utility of certain assays used to evaluate for the presence of a lupus anticoagulant.

Preanalytical factors have a profound effect on sample integrity

The effects of preanalytical factors can account for up to 70 percent of a clinical laboratory’s errors.1

Preanalytical factors influencing the reliability of laboratory test results are commonplace. It is traditionally accepted that hemolyzed samples are unsuitable for coagulation assays because of the release of hemoglobin, intracellular components, and thromboplastic substances from damaged blood cells. The complexity of coagulation reactions and the sophistication of coagulation analytical methods render coagulation testing among the most prone to pre-analytical errors in the clinical laboratory. Hemolysis and under-filling tubes are two of the most common reasons for rejecting specimens. Furthermore, the incidence of hemolysis has increased with the decentralization of phlebotomy practices to non-laboratory personnel.

Program Description
On behalf of the 2010 Planning Committee, we are pleased to announce the program for the joint
HTRS/NASCOLA Scientific Symposium. Based on the overwhelming success of our 2008 collaboration, the Hemophilia and Thrombosis Research Society (HTRS at www.htrs.org) and the North American Specialized Coagulation Laboratory Association (NASCOLA at www.nascola.com) have once again joined forces to create an exciting program with a mixture of symposia covering clinical, basic science, and laboratory topics in hemostasis and thrombosis.

This year's program features simultaneous symposia, as well as special sessions for nurses. Eight symposia will be presented in 6 time slots (4 stand-alone symposia and 4 running concurrently). We have incorporated 2 breakout symposia chaired by the nursing representatives on our planning committee. All HTRS members should encourage their nurses and physician assistants to attend and take advantage of this specialized content.

Lastly, we are honored to welcome 2 outstanding international speakers to this year's meeting. Dr. Alessandro Gringeri is a noted authority on hemophilia who will speak not only during the symposia but also at the Friday night social event. Dr. Guido Finazzi is an expert on thrombosis in myeloproliferative disorders and an excellent speaker as well. We hope you will join us in welcoming these speakers along with our entire distinguished faculty. We look forward to a very productive and stimulating meeting.

1.0 NATT Announces Two Events for March - Blhttp://www.stoptheclot.org/contact.htmood Clot Awareness Month
The National Alliance for Thrombosis and Thrombophilia (NATT) teams with the Association of Black Women Attorneys (ABWA) on a 5K walk to raise awareness about blood clots, particularly within the African American population. ABWA's Community Service Committee chose to sponsor this 5K walk on Saturday, March 20, 2010 because March is Blood Clot Awareness Month. The walk begins at 11:00 AM, in Prospect Park Brooklyn, although "Check-In" begins at 10:00 AM at the Bartel Pritchard Square entrance at Prospect Park West and 15th Street in Brooklyn.
Georgia's Boutique in Soho, NY, on Thursday, March 25, 2010 from 6:00-9:00 PM is hosting a reception to celebrate the success of the walk and to continue the effort to fundraise for NATT and alert attendees about the risk of blood clots. We are honored that Dr. Sharif Ellozy, a vascular surgeon from Mount Sinai Medical Center, is scheduled to speak at this event.

Fluid flow affects: platelet fluxes to the wall, the time available to form bonds, force loading of bonds, cellular deformation, endothelial metabolism, and removal of reactive species created during thrombotic events. We have deployed a variety of techniques in microfluidics and combinatorial high throughput phenotyping of platelet and coagulation function to drive hierarchical Systems Biology models of platelet metabolism and thrombus buildup under flow.

The American Society of ExtraCorporeal Technology is proud to announce the 17th Annual Symposium on New Advances in Blood Management. The meeting is dedicated to presenting the latest information on the field of perioperative blood management and its impact on patient care.
Major subject areas for the meeting include the following:
- Blood Management in Cardiac Thoracic and Vascular Surgery
- Minimizing Bleeding and Allogeneic Blood Transfusion Exposure
- The Role of Extracorporeal Interventions on Optimizing Perfusion and the Improvement of Cardiopulmonary Bypass
- Advances in Coagulation Monitoring: Present and the Future
- Mechanisms of Reducing Inflammation during Cardiopulmonary Bypass
- Autologous Platelet Harvesting and its Applications
Each session will have a minimum of three scientific sessions followed by a question and answer session. Speakers have been chosen who reflect the current state-of-the-art for all content areas and will report on both the physiological aspects of their subjects and on clinical studies. Emphasis will be placed on those interventions that have been scientifically shown, through techniques of evidenced-based medicine, to promote enhanced patient outcome.
This year the meeting will be held from October 1-3, 2009 at the Westin Crown Center in Kansas City, Missouri. Registration information can be obtained below.

The BBTS Annual Conference for 2009 continues the theme from 2008 of providing an invigorating mix of topics from research in the blood centre to practical bedside transfusion. Plenary sessions include managing blood supply in both the developed and the less developed world, transfusion policies in major trauma; civilian and military including problems with ABO blood groups in transplantation and malaria. Parallel sessions range from the latest applications of genetics and stem cell culture to practical aspects of paediatric and platelet transfusion. The meeting should provide something for everyone with an interest in transfusion with many exciting talks by leaders and experts in the field. The conference will be held in the centre of Manchester and the social programme promises to make best use of the active night life of this bustling city!

Workshop on Interdisciplinary Biomedical Research University of Notre Dame
March 1 - 3, 2009 (Sunday - Tuesday), Center for Continuing Education, University of Notre Dame Complex biomedical modeling efforts include predictive multiscale simulations consisting of different submodels at each scale, scalable parallelism for heterogeneous dynamical simulations, a data-rich environment with experimentally determined model parameters and, finally, distributed multidisciplinary research teams and resources. The goal of this workshop is to bring together researchers in biomedical field, biochemistry, bioengineering, biology, computational and mathematical biology and biophysics to discuss recent developments in biomedical modeling and experiment as well as current and possible future collaborations. Some of the topics to be discussed are: thrombus formation, bioimaging, study of osteoporosis.

Conference Description
This conference provides a unique international forum for communication and discussion of current issues in quality and standardization of coagulation diagnostic laboratory testing. Continued conference features include abstract presentations (oral and poster) and Breakfast-with-the-Experts sessions. There will also be presentations by expert faculty on a wide range of topics, along with panel discussions and question and answer sessions.
Conference Learning Objectives
Upon completion of the conference, participants should be able to:
- Describe general aspects of planning and management of coagulation laboratory testing
- Provide examples of processes and goals of quality assessment and improvement processes
- Discuss the complexities of testing for disorders of platelets and platelet function
- Summarize testing quality issues for diagnosis and treatment of von Willebrand disease
- Review the diagnostic and quality challenges associated with testing for factor VIII inhibitors
Workshop Description
New this year - Two workshop sessions will be conducted Wednesday and Thursday afternoons. The first will be focused on testing for platelet functional disorders, and the second will focus on testing for selected plasmatic coagulation disorders. Each workshop session will begin with participating companies presenting information on specific laboratory instrument and/or reagent testing. Interactive case studies by conference faculty on thematic topics will conclude each workshop session.
Workshop Learning Objectives
Upon completion of the workshop, participants should be able to:
- Comprehend the role of the coagulation diagnostics industry in achieving high-quality testing
- Provide specific examples of platelet hypofunctional disorders and testing procedures for them
- Recount examples of specific complex coagulation disorders and quality of diagnostic testing
